Collection Notice

Sentinelle Gestion Parasitaire Inc. collects, through this online form, the information you provide to review your situation, respond to your request, communicate with you and, when applicable, prepare and follow up on a service proposal. Photographs are optional; visible content and metadata attached to the files may also be collected.

The information may be accessed by authorized SGP officers and technical service providers that support the form, hosting and professional email. These providers may process or store information outside Québec, where privacy rules may differ. You may request access or correction, withdraw consent subject to legal and contractual limits, or make a complaint. See the Privacy Policy.

Do not include people, documents, address labels, licence plates or information concerning another person. Remove location data from the file where possible and attach only photographs you are authorized to provide.

This form is intended for people aged 14 or over; otherwise, a parent or guardian must submit the request.
